You are authorizing GekkoTech Computer Services to perform diagnostic and repair work to your computer, phone or tablet. GekkoTech Computer Service's Technicians are trained professionals in performing technical diagnostics and repair work to your device. GekkoTech is not liable for any hardware failures beyond our control. If your device contains any 3rd party parts, and these parts are damaged as a result of diagnostics or repair by GekkoTech, GekkoTech will not be held liable for the cost of replacing the damaged 3rd party parts. Data backup is the responsibility of the owner of the device and GekkoTech is not liable for any loss of data incurred from performing repair and diagnostics work to your device. You agree that you are the owner of the device being repaired and are responsible for paying for all incurred charges. Failure to pay for any repair work, will result in your device being held until payment is received. A diagnostic fee of $49.99+tax will apply on any device checked in for repair, in which the repair quote is declined. In the event that a device is not picked up within 14 days of being notified that your device is ready for pickup, a late fee of 10% of repair quote, or $20, whichever is greater, plus applicable taxes will apply. After 20 days of not being picked up, ownership of the repaired device will be relinquished by you and transferred to GekkoTech Computer Services. No work will be performed until discussing all estimated charges with you. Your device service might be covered by warranty, consumer law, or AppleCare+. The following requested personal information (name, address, telephone number and email address) are necessary to proceed with the request for service. If the service is requested pursuant to a third party service obligation, such information will be transferred to the third party for verification and quality purposes, including confirming the service transaction and submitting a customer survey. By signing our work authorization form in-store, you consent to this transfer and the use by the third party for this purpose and agree to the above terms of service.
